Known for its enormous cast and equally high fatality rate, Grey's is also known for every episode title being taken from the name of a song. The series currently has 362 episodes over 16 seasons, with the 17th planned.

After the plane crash in "Flight" (S08E24), the survivors attempt to use an Orion Flare Gun to signal a nearby search plane, but the flare gun fails to fire.

While never actually seen, an M9A1 Bazooka provides the inciting incident for "It's the End Of The World.../As We Know It" (S02E16 and 17) after its projectile becomes lodged in a man's chest.
Meredith extracts the bazooka round from the man's chest in "As We Know It" (S02E17). The head is a propperly marked M6A1 warhead, which shouldn't be compatible with the M9A1, but then you also shouldn't build live explosive weapons and then stand in front of them.
At the end of the episode a whiteboard is seen with various pictures and sketchs from the bomb squad trying to determine their best move. None of the pictured weapons are actual 2.36" bazooka rounds, though the 3.5" M28 rocket at top is related.
